Andrologische Diagnostik<\/h1>

Vor jeder Therapie wird bei uns ein Basisspermiogramm angefertigt. Hier werden Anzahl, Beweglichkeit und Intaktheit der Spermien \u00fcberpr\u00fcft.<\/p>

F\u00fcr die Therapieentscheidung kann es wichtig sein, eine Aufbereitung der Spermaprobe durchzuf\u00fchren. Dies erm\u00f6glicht die Beurteilung der Langzeitbefruchtungsf\u00e4higkeit der Spermien.<\/p>

Kryokonservierung von unbefruchteten Eizellen<\/a><\/h3>

Die Fruchtbarkeit zu sch\u00fctzen ist heute ebenso f\u00fcr Frauen m\u00f6glich, auch wenn das Eizellbanking noch nicht sehr gel\u00e4ufig ist. Das liegt daran, dass diese Methode aufwendiger als bei M\u00e4nnern ist und erst in den letzten Jahren experimentell zur Anwendung gebracht wurde. Man ben\u00f6tigt hierf\u00fcr eine hormonelle Vorbehandlung von ca. 14 Tagen und eine Entnahme der Eizellen durch Follikelpunktion.<\/p>

Nach Kryokonservierung und Lagerung in fl\u00fcssigem Stickstoff bei \u2013196 \u00b0C stehen diese sp\u00e4ter f\u00fcr eine k\u00fcnstliche Befruchtung (IVF) zur Verf\u00fcgung. Unbefruchtete Eizellen \u00fcberleben das Einfrieren und Auftauen dank moderner Einfriermethoden (Vitrifikation) fast wie befruchtete Eizellen. Wenn die \u00fcberlebenden Eizellen einer k\u00fcnstlichen Befruchtung zugef\u00fchrt werden, kann man damit rechnen, dass ca. die H\u00e4lfte der Eizellen befruchtet wird.<\/p>

Ein mehrstufiges Sicherheitssystem sch\u00fctzt vor Verwechslungen und Verlust.<\/p>

Weltweit sind mehr als 100 Geburten nach dem Einfrieren unbefruchteter Eizellen beschrieben. Auch wir bieten Ihnen bei Bedarf die Vitrifikation von Eizellen an. Alternativ gibt es auch die M\u00f6glichkeit Eierstockgewebe einzufrieren, \u00fcber die wir Sie gerne beraten werden.<\/p>

Kryokonservierung befruchteter Eizellen<\/a><\/h3>

Wenn die erkrankte Frau in einer festen Partnerschaft lebt, ist das Einfrieren von befruchteten Eizellen nach einer daf\u00fcr notwendigen hormonellen Stimulation und Eizellentnahme in Allgemeinnarkose eine sehr gute Option. Eingefroren werden die befruchteten Eizellen im \u201eVorkernstadium\u201c, d.h. einen Tag nach der Follikelpunktion, bevor es zur Verschmelzung von m\u00e4nnlichem und weiblichem Chromosomensatz gekommen ist. Notwendig ist daf\u00fcr ein technisch aufwendiges, langsames Herunterk\u00fchlen der Eizellen, um die bei jedem Einfriervorgang entstehende Kristallbildung m\u00f6glichst gering zu halten.<\/p><\/div>

Das Einfrieren und Auftauen \u00fcberleben befruchtete Eizellen mit einer ca. 80%igen Chance. F\u00fcr den Fall, dass nach der Behandlung der Grunderkrankung die Fruchtbarkeit nachhaltig Schaden genommen hat, kann eine Schwangerschaft dann mit Hilfe der zuvor eingelagerten befruchteten Eizellen herbeigef\u00fchrt werden.<\/p>

Wichtig ist, dass diese Methode nur angewandt werden darf, wenn die Partnerschaft noch fortbesteht und beide Partner dem Embryotransfer zustimmen. Diese Prozedur ist aus Sicht der Frau und unter dem Aspekt der Vorsorge daher nur sinnvoll, wenn die Partnerschaft fortdauert.<\/p>

Kryokonservierung von Samenzellen<\/a><\/h3>

Ebenso k\u00f6nnen Samenzellen aus der Samenfl\u00fcssigkeit oder aus dem Hodengewebe kryokonserviert und aufbewahrt werden. Unabh\u00e4ngig von einer bereits begonnenen Sterilit\u00e4tstherapie besteht die M\u00f6glichkeit Samenproben bei uns einfrieren zu lassen, wenn die Gefahr einer Fruchtbarkeitssch\u00e4digung droht. Dies ist z. B. bei M\u00e4nnern der Fall, die aufgrund einer Krebserkrankung eine Chemo- oder Strahlentherapie durchf\u00fchren lassen m\u00fcssen. Chemo- und Strahlentherapien k\u00f6nnen das Hodengewebe so sch\u00e4digen, dass nach Heilung der Krebserkrankung keine Spermien mehr in der Samenfl\u00fcssigkeit nachweisbar sind.<\/p>

Wenn zuvor Spermien kryokonserviert wurden (Spermabanking) und nach einer erfolgreichen Krebsbehandlung die Zeugungsf\u00e4higkeit nachhaltig eingeb\u00fc\u00dft sein sollte, kann mit Hilfe des vor der Behandlung angelegten Samendepots noch viele Jahre sp\u00e4ter ein Kind gezeugt werden.
In manchen F\u00e4llen ist dann zwar die Hilfe eines Reproduktionsmediziners erforderlich, aber eine leibliche Vaterschaft ist gut m\u00f6glich.<\/p>

Nach Kryokonservierung von Spermien empfehlen wir ein Kontrollspermiogramm ca. ein halbes Jahr nach abgeschlossener Therapie. Wenn die Spermienproduktion erhalten geblieben ist, k\u00f6nnen die eingefrorenen Proben aufgetaut und verworfen werden.<\/p>

Samenzellen k\u00f6nnen auch dann auf Wunsch eingefroren werden, wenn der Mann beruflich z. B. viel auf Reisen ist und eine Anwesenheit am Tag der Eizellentnahme seiner Frau nicht m\u00f6glich ist. As part of a test cycle at the beginning of your menstral cycle (between the second and fourth day) we check the hormones that influence the maturation of the egg cells. That includes hormones produced by the hypophysis, the thyroid, the ovaries and the adrenal. Due to the fact that the hormone levels vary during the course of your menstral cycle, it is essential that the blood test is perfomed at a specific point in time at the beginning of the cycle.<\/p>

Should this basal blood sampling show certain irregularities then additional functions will be tested, in order to determine the cause of the problem.<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t

It is often useful to find out when and if ovulation is taking place. We can do this by means of ultrasonic scanning and hormone tests during the course of your cycle. This will also show us any changes in your hormones levels, which are important factors influencing egg cell quality. As part of this process the corpus luteum hormone will also be tested. This hormone is responsible for providing good nidation conditions in the second half of the cycle.<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t

In the days leading up to ovulation the mucus in the cervix changes. It becomes clear, thinner, more fluid, and stretchy. This allows the sperm cells to easily make their way to the uterine cavity.<\/p>

As part of a post-coital test (an examination at the moment of ovulation and the morning after intercourse), mucus is withdrawn from the cervix and examined<\/p>

The quality of the mucus will be evaluated as well as the microscopic evidence of sperm in the mucus sample. If many healthy sperms are found, the chances of becoming pregnant in this manner is equally as high as by means of artificial insemination.<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t

General information<\/strong><\/p>

The success of embryonic nidation depends greatly on the whether the interior of the uterus is in optimal condition.<\/p>

A hysteroscopy is a simple procedure which allows us to rule out any abnormalities inside the uterus. Thinnest optical devices are used so that this procedure can be performed without requiring any anaesthesia. Nevertheless, if you prefer, a short anaesthesia is available.<\/p>

The results of the examination will be shared with you immediately following the procedure. This allows us to decide quickly if any further therapeutic procedures are necessary.<\/p>

The procedure takes approximately 5 minutes. If no anaesthesia is necessary, as planned, you will be able to continue your daily routine. However, it is possible to receive a doctor’s note if requested.<\/p>
